The Flinders Program Implementation team provides organisations across Australia with mentoring support to implement Chronic Care Management into the patient delivery systems. Currently the team is working with up to 50 organisations across Australia with representation from each state and territory.
The Implementation team provides organisations with support and mentorship to develop:
- Chronic Care model and service delivery pathways
- Conduct a full system audit and health systems assessment
- Skills audit and process mapping
- Develop an organisational plan
- Pilot and evaluate the plan
- Develop policy & procedure
- Support a sustainable workforce skill base in chronic condition self-management
- Support governance structures; to ensure workforce capabilities and skills are reflected in position descriptions, client care delivery and staff orientation and annual reviews
- On-going support; provide ongoing evaluation, quality improvement audits and refresher day skills
