For over two decades The Flinders Program team has delivered training in chronic condition management and risk factor management, based on the underlying principles of self management.

A range of training options are available across Australia and internationally, online and face-to-face.

The Flinders Program training options

We offer both face to face and online options for the Flinders Chronic Condition Management (CCM) Program. Both our workshops and our online courses have been designed to cater for health professionals who work in mainstream chronic condition management and/or with Aboriginal and Torres Strait Islander clients.

Delivery Options

  • 2 day workshop at your location
  • 2 day workshop in Adelaide;

  • Online equivalent of the 2 day workshop. The online courses are presented as a series, with each designed to take an average 30-90 minutes for the online learner. There are additional options for those working with clients with specific needs around tobacco and smoking 


Certificates
A Certificate of Completion is awarded to participants who complete the training. This certificate entitles you to use The Flinders Program in your clinical practice.

A Certificate of Competence is awarded to participants who, in addition to completing the training, successfully complete one or more Care Plans for review by the Flinders Accredited Trainer.

Following the training support is provided, enabling you to use the Flinders tools in a productive way in your workplace.

What happens next?
Having completed the Flinders Chronic Condition Management (CCM) Program you can choose to continue into either the Accredited Trainers’ program, or into post-graduate studies in chronic condition management.

The Flinders Accredited Trainer Program is for participants who have completed The Flinders Program training and who intend to train others in this.

Completion of The Flinders Program training is recognised within the post-graduate chronic condition management programs offered at Flinders University.

Completion of Certificate of Competency in the Flinders Living Well Smoke Free program can be used to support an application for recognition for prior learning from your registered training organisation (RTO) delivering learning outcomes (competencies) in the following vocational education sector’s training units:

  • HLTPOP403C – Provide information on smoking and smoking cessation
  • HLTPOP404C – Provide interventions to clients who are nicotine dependent

The Flinders Chronic Condition Management Program is accredited with professional bodies including

RACGP and ACRRM

It is also accredited by Flinders University Academic Board for units in the Graduate Certificate in Health Chronic Conditions, Graduate Diploma in Chronic Condition Management and Masters in Public Health Chronic Conditions.

 

Recognition for Prior Learning

Vocational Education Training (VET) competencies

A Certificate of Competency in the Flinders Closing the Gap Program can be used to support an application for recognition for prior learning from your registered training organisation (RTO) delivering learning outcomes (competencies) in the following vocational education sector’s training units:

  • CHCICS406B – Support client self- management
  • CHCICS407B – Support positive lifestyle 
  • CHCICS408B – Provide support to people with chronic disease
